Responsabilidad profesional del odontólogo frente a la sospecha de maltrato infantil: revisión sistemática de la evidencia clínica, ética y jurídica

Introducción: El maltrato infantil es un problema de salud pública en el que el odontólogo ocupa una posición importante para la detección temprana debido a la elevada frecuencia de lesiones en cabeza, cuello y cavidad oral. Sin embargo, la evidencia sobre su responsabilidad jurídica permanece dispersa entre publicaciones bioéticas y odontológico-forenses. Objetivo: Sintetizar la evidencia científica disponible sobre la responsabilidad jurídica del odontólogo frente a la sospecha de maltrato infantil, analizando sus obligaciones profesionales y las implicaciones derivadas de su actuación u omisión. Métodos: Se realizó una revisión sistemática conforme a la declaración PRISMA 2020. La búsqueda se efectuó en PubMed, Scopus y ProQuest mediante estrategias basadas en términos MeSH, DeCS y palabras libres. La selección de estudios se realizó de forma independiente por dos revisores utilizando Rayyan. Resultados: De los 78 registros identificados, 11 estudios cumplieron los criterios de elegibilidad. La evidencia se organizó en tres dominios: identificación del maltrato infantil, obligaciones éticas y jurídicas del odontólogo, y barreras para la actuación profesional. Los estudios coinciden en el deber de identificar y notificar las sospechas de maltrato. No obstante, persiste una brecha entre la detección clínica y la notificación formal, relacionada principalmente con el diagnóstico y la falta de capacitación.Conclusiones: La responsabilidad del odontólogo comprende el tratamiento clínico y la actuación diligente para proteger integralmente al niño. Reducir la brecha entre la sospecha y la activación oportuna de los mecanismos de protección requiere fortalecer la formación jurídica, establecer protocolos institucionales y mejorar la coordinación interinstitucional.
