Trastornos del sueño y enfermedades neurológicas: revisión de la evidencia actual

Resumen

Introducción: Los trastornos del sueño constituyen una manifestación clínica frecuente en diversas enfermedades neurológicas y pueden estar asociadas con deterioro del comportamiento durante el día, sintomatología más evidente, empeoramiento de la calidad de vida y peor evolución clínica. Alteraciones tales como: insomnio, somnolencia diurna excesiva, trastorno de conducta del sueño REM, síndrome de piernas inquietas, movimientos periódicos de las extremidades y trastornos respiratorios presentan características diferentes según la enfermedad neurológica y el método utilizado para su evaluación.

Objetivo:  Analizar críticamente la evidencia científica reciente acerca de la relación entre los trastornos del sueño y las principales enfermedades neurológicas del adulto.

Metodología: Se llevó a cabo una revisión sistemática siguiendo los lineamientos PRISMA 2020. La búsqueda se concentró en PubMed/MEDLINE, Scopus, ScienceDirect y SciELO, incluyendo estudios primarios publicados entre 2021 y 2026 en inglés y español. Se seleccionaron investigaciones en adultos que evaluaran el sueño mediante polisomnografía, poligrafía respiratoria, actigrafía, criterios diagnósticos reconocidos o cuestionarios validados. Tras el proceso de identificación, cribado y elegibilidad, se incluyeron 23 estudios.

Resultados: La evidencia recabada mostró una elevada concordancia clínica y metodológica. En la enfermedad de Parkinson destacaron la mala calidad del sueño, la somnolencia diurna y el trastorno de conducta del sueño REM; en Alzheimer, el insomnio y la desorganización del ritmo sueño-vigilia; en epilepsia, la mala calidad del sueño y su impacto sobre la calidad de vida; en esclerosis múltiple, el insomnio y el síndrome de piernas inquietas; y en ictus, los trastornos respiratorios, el insomnio y su asociación con el pronóstico. Las intervenciones farmacológicas con melatonina, lemborexant y no farmacológicas con exergaming mostraron beneficios específicos, aunque la evidencia terapéutica fue limitada.

Conclusiones: Los trastornos del sueño deben tomarse en cuenta en la valoración integral del paciente neurológico. Se requieren estudios longitudinales, multicéntricos y ensayos clínicos con criterios diagnósticos y desenlaces claros.
