Síndrome mediastínico como presentación de linfoma de Hodgkin clásico, subtipo esclerosis nodular, variante sincitial, en una mujer joven

Resumen

La variante sincitial del linfoma de Hodgkin clásico, subtipo esclerosis nodular, es infrecuente y puede presentarse como masa mediastínica bulky con importante dificultad diagnóstica. El o. Se realizó un reporte de caso retrospectivo basado en historia clínica, estudios de laboratorio, radiografía, tomografía, resonancia magnética e informe histopatológico, complementando con una revisión narrativa de la literatura sobre masas mediastínicas en adultos jóvenes. El caso corresponde a una mujer de 21 años con fiebre, pérdida de peso, poliartralgia, edema facial y periférico, tos y ensanchamiento mediastínico. El abordaje inicial se orientó a patología vascular por diferencial tensional y signos compresivos. La tomografía evidenció masa mediastínica izquierda bulky con infiltración pulmonar, derrame pleural y derrame pericárdico. La resonancia descartó origen vascular primario y mostró necrosis central. La toracotomía con biopsia informó linfoma de Hodgkin clásico, subtipo esclerosis nodular, variante sincitial, grado 2. Como limitaciones no se realizó inmunohistoquímica y la paciente falleció antes de la estadificación formal y del tratamiento específico. Se concluye que el síndrome mediastínico con simulación vascular puede retrasar el diagnóstico del linfoma de Hodgkin; en este contexto, la obtención oportuna de tejido representativo mediante biopsia amplia es decisiva para orientar el diagnóstico y la oportunidad terapéutica en presentaciones atípicas.
