Novel Calcitonin Gene-Related Peptide (CGRP)-Targeted Therapies for Migraine: A Review of Current Evidence

Abstract

Introduction: Migraine is a highly prevalent neurological disorder and one of the leading causes of disability worldwide. The identification of calcitonin gene-related peptide (CGRP) as a key mediator in its pathophysiology has driven the development of targeted therapies that have demonstrated promising results in the prevention and treatment of this condition.

Objective: To analyze the recent scientific evidence regarding the efficacy and safety of therapies targeting the calcitonin gene-related peptide (CGRP) pathway for the prevention and treatment of migraine.

Methods: A narrative literature review was conducted by searching for studies published between 2021 and 2026 in the PubMed, Google Scholar, and ScienceDirect databases. Systematic reviews, meta-analyses and clinical practice guidelines evaluating the efficacy and safety of therapies targeting the CGRP pathway in adult patients with migraine were included.

Results: The reviewed studies show that therapies targeting the CGRP pathway, including monoclonal antibodies and gepants, have demonstrated efficacy in reducing the frequency and intensity of migraine attacks, improving quality of life, and presenting a favorable safety profile. However, variability in clinical response, cost, and the need for further long-term evidence remain important challenges, promoting the search for more personalized therapeutic strategies.

Conclusion: Targeted therapies against the CGRP pathway represent a significant advance in migraine treatment, demonstrating efficacy, safety, and a more specific therapeutic approach compared with conventional treatments. However, variability in clinical response and access limitations highlight the need for continued research to develop strategies that enable a more personalized treatment approach.
