Resistencia antimicrobiana en infecciones urinarias pediátricas: patrones de resistencia, desafíos terapéuticos y estrategias de optimización del tratamiento antibiótico

Introducción: Las infecciones de vías urinarias (IVU) forman parte de una de las infecciones bacterianas que se presentan frecuentemente en la edad pediátrica y llegan a ser una de las principales causas de consulta médica, así como el uso de antibióticos y hospitalización; En diversas investigaciones de los últimos años se ha evidenciado un incremento de la resistencia antimicrobiana, especialmente en patógenos como E. coli, lo que ha generado dificultades dentro del tratamiento empírico, lo que ha generado un aumento en las tasas del riesgo al fracaso terapéutico, así como la recurrencia de infecciones y las complicaciones renales a largo plazo. Objetivo del trabajo: Analizar la resistencia bacteriana en infecciones de vías urinarias en la población pediátrica, llegando a identificar los principales microorganismos implicados más frecuentes, así como los factores que se asocian al desarrollo de la infección, el impacto del diagnóstico y tratamiento. Metodología: Se realizó una revisión bibliográfica narrativa por medio de artículos científicos publicados en bases de datos como Scopus, SciELO, Google Scholar, PubMed. Dentro de la revisión se incluyeron estudios originales, así como revisiones bibliográficas y artículos científicos que han sido relacionados con infecciones urinarias pediátricas, la resistencia antimicrobiana, el tratamiento en niños con infección de vías urinarias y los mecanismos bacterianos de resistencia. Se ha seleccionado publicaciones de los últimos 5 años que tengan relevancia clínica y científica. Principales hallazgos: Se evidencio que Escherichia coli es el principal agente etiológico de las IVU tanto de la población adulta, como la población pediátrica, así también, se ha evidenciado un aumento progresivo de cepas resistentes a antibióticos que se utilizan comúnmente como ampicilina, trimetoprim/ sulfametoxazol y cefalosporinas principalmente de 3ra generación. La producción de betalactamasas de espectro extendido (BLEE) es uno de los principales mecanismos de resistencia por la capacidad para limitar las opciones terapéuticas. Otros factores asociados es el uso de antibióticos, infecciones recurrentes, hospitalización y anomalías congénitas del tracto urinario. Conclusión: La resistencia antimicrobiana es un problema ante el manejo de infecciones urinarias pediátricas, se han evidenciado importantes repercusiones clínicas, por tanto, la vigilancia microbiológica, el uso racional de antibióticos y el ajustar el tratamiento ante los factores que presente cada paciente son estrategias que se deben cumplir para mejorar la recuperación y el pronóstico del paciente.
